Volunteers of America Oregon seeks a Licensed Practical Nurse. The Licensed Practical Nurse provides oversight of all nursing services including chronic care management, oversight of the medication program, training, and supporting the RN and medication staff, CNA’s and other direct care staff of two adult day centers. The Licensed Practical Nurse also participates in the care plan process, provides communication and observation insights of participants with other members of the health care team, including physicians, therapists and case managers.
