The Coalition of Community Health Clinics (CCHC) improves healthcare for vulnerable populations residing in the Portland metro area by strengthening community health clinics, promoting health equity, and fostering collaboration across the safety-net delivery system.

The Health and Housing Coordinator provides an opportunity to help shape and launch an innovative new program to bridge the divide between housing services and healthcare and improve care for medically vulnerable individuals. The position will work in a fast-paced environment of health transformation with our team of five staff, 15 nonprofit clinic members and the Mobile Permanent Supportive Housing Team (Mobile PSH Team). The position will work directly with case managers and their medically vulnerable clients to ensure they have access to medically, geographically, and culturally appropriate healthcare. The Health and Housing Coordinator will work with CCHC’s Deputy Director to help develop referral and service coordination processes that support long-term program implementation.
