Yamhill CCO request for proposals invites projects addressing health and prevention

$1.5 million in ‘Build Forward’ grant funding available

McMINNVILLE, Ore. — Yamhill Community Care Organization is pleased to announce a new round of grant funding called Build Forward. The funding will support projects focused on improving the health of the community in new and innovative ways.

Proposals should align with the Triple Aim in health care delivery (better health outcomes, high quality of care and reduced costs), Yamhill CCO’s existing plans to implement health care transformation, and Yamhill CCO’s vision: “A unified healthy community that celebrates physical, mental, emotional, spiritual and social well-being.”

Approximately $1.5 million will be available for all the approved projects. Funding will be for one year. To apply, organizations must provide services within the Yamhill CCO service area, but do not have to be a hospital, clinic or other health care provider.

In order for proposals to be considered, they must address at least one objective in one of Yamhill CCO’s existing implementation plans (Transformation Plan, Community Health Improvement Plan, and CCO and Early Learning Hub Strategic Plans), which can be found at www.yamhillcco.org/about-us. In addition, those submitting proposals are strongly encouraged to attend a workshop or schedule a phone call to review proposals for alignment with Yamhill CCO objectives.

Past funding for Invest Forward and Transform Forward grants has gone toward projects like the Student Nutrition and Activity Clinic for Kids (SNACK program), which supports youth and their families in making healthy lifestyle choices; Community EMS, which brings medical care to rural areas; and the Community Health Hub at Yamhill CCO, which offers resource coordination to high-need members.
