The Oregon Chapter of Healthcare Information and Management Systems Society presents national thought leaders, Rachel Gold, PhD, MPH, Lead Research Scientist at OCHIN, Inc., and Briar Ertz-Berger, MD, MPH, Assistant Director of Operations for Special Populations, Kaiser, in an interactive discussion on their research and their organizations’ journeys incorporating social determinants of health into electronic health records. There is a growing awareness that social determinants of health -- the non-clinical circumstances of patients' lives – can profoundly impact health. However, guidelines and best practices on how health care providers can or should integrate social determinants of health into their electronic health records and workflows are in their early stages.
At this Population Health Management event, Gold will present results from her team's research on strategies for using electronic health records to document patients' social determinants of health in community health centers, and approaches to helping primary care clinics adopt social determinants of health documentation. Additionally, Ertz-Berger, will discuss Kaiser Permanente’s journey to implement total health assessments using a social determinants of health screening tool, making social diagnoses, and an overview of the National work to deploy a Social Services Resource Locator.
