Lane County should be a healthy place to live, learn, work and play. Trillium Community Health Plan, Lane County, PeaceHealth and United Way invite the community to help create a plan that will provide a healthier future for Lane County. The goal of this process is to identify key priority areas where the community can take action to improve population health. The findings from the community health assessment will lead to the creation and implementation of the 2016-2019 Community Health Improvement Plan (CHIP).

In addition to the visioning session, the 2015 Community Health Needs Assessment will include four different

community assessments:

1-Community Health Status Assessment: What does the health status of our community look like?

2-Local Public Health System Assessment: What are the components, activities, competencies, and capacities of our local public health system? How are the Essential Services being provided to our community?

3-Community Strengths and Themes Assessment: What is important to our community and how is quality of life perceived? What assets do we have that can be used to improve community health?

4-Forces of Change Assessment: What is occurring or might occur that affects the health of our community or the local public health system? What specific threats or opportunities are generated by these occurrences?
