Oregon Announces Healthcare Patient Safety Leaders

Oregon Patient Safety Commission Honors Reporting Program Participants

Portland, OR — On February 26, the Oregon Patient Safety Commission (OPSC) will honor exemplary healthcare facilities at the Fifth Annual Oregon Patient Safety Forum. The facilities being recognized are leading participants in Oregon’s Patient Safety Reporting Program—a program designed to understand why and how patients are harmed during medical care and to share successful strategies for making care safer. Rather than simply measuring the number of events reported, the reporting program focuses on learning from the events that cause patient harm. In 2011, OPSC established recognition targets to guide facilities participating in the reporting program and to ensure that enough reports are received to build a strong database for shared learning. For the 2015 calendar year, eight healthcare facilities are receiving the OPSC’s Leading Participant Award for consistently investigating patient harm, developing solutions to prevent future patient harm, and submitting reports that effectively contribute to shared learning across Oregon. The 2015 Patient Safety Reporting Program Leading Participants are:

  • Corvallis Clinic Surgery Center (Corvallis)
  • Maryville (Beaverton)
  • PeaceHealth Sacred Heart Medical Center University District (Eugene)
  • Providence Child Center (Portland)
  • Providence Portland Medical Center (Portland)
  • South Hills Rehabilitation Center (Eugene)
  • St. Charles Madras (Madras)
  • Tuality Community Hospital (Hillsboro)

Three healthcare corporations will also be recognized for their exemplary organizational commitment to the reporting program and patient safety in 2015. The healthcare corporations receiving special recognition are:

  • Kaiser Permanente
  • Marquis Companies
  • Providence Health and Services

The event will also be marked by recognition of an additional 61 facilities that met or exceeded the 2015 recognition targets. Lists of all ambulatory surgery centers, hospitals, nursing facilities, and pharmacies that met or exceeded the 2015 reporting targets are available on OPSC's website, oregonpatientsafety.org. "The event is a great opportunity to acknowledge facilities that are truly putting forth an effort to improve, and to inspire healthcare facilities to strive for continued excellence," says the OPSC's Executive Director Bethany Walmsley. "Being recognized for active participation in the reporting program does not mean that these facilities experience more patient harm than others. Rather, their reporting shows a dedication to the learning and transparency that is necessary to improve patient safety." About the Oregon Patient Safety Commission OPSC is a semi-independent state agency charged by the Oregon Legislature with reducing the risk of serious adverse events occurring in Oregon’s healthcare system and encouraging a culture of patient safety. OPSC operates the Patient Safety Reporting Program, offers Oregon healthcare organizations a variety of opportunities to participate in infection prevention trainings and collaboratives, and is the administrative entity for Early Discussion and Resolution—Oregon’s voluntary process for open conversation between healthcare providers and patients, if serious physical injury or death occurs during healthcare.
