OPSC Honors Oregon’s Patient Safety Leaders

Honorees Will Be Recognized at the Sixth Annual Oregon Patient Safety Forum on March 3

Portland, OR—The Oregon Patient Safety Commission (OPSC) is proud to convene the Sixth Annual Oregon Patient Safety Forum on March 3. This occasion recognizes some of the strong patient safety work undertaken in 2016 by healthcare organizations across Oregon, and will generate momentum for continued action to improve patient safety. Honorees at this year’s event include healthcare organizations that have actively engaged with Oregon’s Patient Safety Reporting Program (PSRP). In addition, this year OPSC will, for the first time, recognize organizations that have made an outstanding commitment to transparent communication with patients and families. The Patient Safety Forum kicks off national Patient Safety Awareness Week and Oregon Patient Safety Awareness Week (March 12-18).

Two national experts on the critical importance of open conversation when healthcare does not go as planned will be featured at this event: Carole Hemmelgarn, a patient safety advocate with a compelling personal story, and Dr. Tom Gallagher, an expert in communication between providers and patients and families following adverse events.

PSRP Honorees
PSRP is a program designed to understand why and how patients are harmed during medical care and to share successful strategies for making care safer. This year’s honorees each exceeded 2016 recognition targets established by OPSC and stood out among their peers.

These healthcare organizations will receive OPSC’s 2016 Leading Participant award for consistently analyzing patient harm events and for their contributions that make shared learning possible:

  • Asante Three Rivers Medical Center
  • Cornerstone Care Option
  • Kaiser Permanente’s Interstate, Skyline, and Sunnybrook Ambulatory Surgery Centers
  • Marquis Newberg
  • PeaceHealth Peace Harbor Medical Center
  • St. Charles Bend

Five healthcare organizations will receive honorable mentions for their contributions to PSRP:

  • Kaiser Westside Medical Center
  • Pacific Surgery Center
  • PeaceHealth Sacred Heart Medical Center at Riverbend
  • Providence Milwaukie Hospital
  • South Hills Rehabilitation Center (Pinnacle)

Two healthcare corporations will receive special recognition for their organizational commitment to shared learning in 2016:

  • Pinnacle Healthcare Management
  • St. Charles Health System

Other healthcare organizations that exceeded or met the 2016 PSRP recognition targets will be acknowledged at the event and are listed on the OPSC website.

Leading Through Transparency Honorees
This year, OPSC is also proud to honor Oregon healthcare organizations that have made an outstanding commitment to transparent communication with patients and families as part of their participation in the first cohort of the Oregon Collaborative on Communication and Resolution Programs (OCCRP). The 2016-17 Leading Through Transparency honorees are:

  • Columbia Memorial Hospital
  • Grande Ronde Hospital
  • Providence St. Vincent Medical Center
  • Salem Health
  • The Oregon Clinic
  • Women’s Healthcare Associates

“This event serves as a reminder that all of us, across the continuum of care, have a role to play in keeping patients safe,” says Melissa Parkerton, OPSC’s Interim Executive Director. “Even when care does not go as planned, open communication may help bring greater peace of mind to patients and providers and improved care for future patients.”
