OCHIN Launches Acuere Health Care Data Aggregation System

Seamless integration and easy navigation help clinicians deliver better care and outcomes while lowering costs

Nonprofit health IT, research, and data innovation organization OCHIN today announced the launch of Acuere, a real-time data aggregation system that allows health care organizations to enhance care delivery and improve business operations.

Acuere is one of the most comprehensive data sets available on the market today, incorporating clinical, claims and social determinant information on more than 10 million patients. More real-time data – and better analysis of that data – allows users to set benchmarks and make better-informed clinical and operational decisions. Acuere’s user-friendly, web-based platform integrates seamlessly with existing electronic health records (EHRs), claims and other information systems. It offers users customizable dashboards and other practical tools that are easy for clinicians and administrators to use and provides one cohesive platform, limiting the need for different toolsets and training.

“Acuere improves patient care by providing real-time updates with an intuitive interface for practitioners,” said OCHIN’s chief executive officer, Abby Sears. “Most importantly, Acuere integrates seamlessly with any EHR or claims system to simplify population health for our vast network of customers and their patients.”

Acuere’s pilot customers saw immediate improvement in patient health outcomes.

“Through the use of Acuere, two of our satellite clinics have achieved better blood pressure control than our organization as a whole: Detroit Shoreway is at 84 percent and The Centers Rocky River is at 90 percent, while our community health centers are at 77 percent overall,” said Marianella Napolitano, vice president of performance improvement at Cleveland, Ohio-based Neighborhood Family Practice. “Being able to share the data at the provider, site and organization level creates an environment of healthy competition resulting in better documentation practices and sharing of ideas to improve performance.”

“Our clinic has used Acuere for several years to support our medical home transformation and clinical quality improvement efforts,” said Erin Kirk, clinic quality manager at Portland, Oregon-based OHSU Family Medicine at Richmond Clinic. “Acuere easily identifies patients with gaps in their care and monitors performance at the primary care physician and care team level. It helps us track performance over time and make sustainable, incremental improvements for positive impacts on the health of our population and our bottom line.”

Acuere lets organizations drill down into data, helping users simplify and operationalize population health and care coordination strategies while leveraging this data to lower costs. Acuere compiles data from EHR and non-EHR sources and provides analytic tools to help organizations make better-informed decisions for measurable improvement. Acuere has ingested and integrated data from claims systems as well as most of the major EHR and EDR vendors, including Epic, Centricity, SuccessEHS, Vitera, NextGen, Dentrix, Intergy, Athena and Greenway.
