Jackson Care Connect Names Ginger Scott, RN, as Transformation Specialist

To support Jackson Care Connect’s ongoing efforts to improve health in Jackson County, the Coordinated Care Organization has named Ginger Scott, RN, as Transformation Specialist. Jackson Care Connect is one of three Coordinated Care Organizations for Oregon Health Plan (Medicaid) members in Jackson County.

“Our goal is to make health care more effective, easy to access and engaging for patients,” said Jennifer Lind, Jackson Care Connect’s regional executive.

In her new role, Ginger will work with community partners to gauge progress for innovative health care projects supported by Jackson Care Connect:

Community health workers at La Clinica and Community Health Center who work one-on-one with Jackson Care Connect members to remove barriers to care. Barriers might include not knowing how to access care,  needing transportation to medical appointments, requiring mental health or addiction services, or needing education about treatment.
Emergency care navigators at Providence Medford Medical Center to assist patients in getting the appropriate care they need.
Opioid Prescribers Group working to develop opioid prescribing standards that help people with chronic pain reduce their dependence on opioids.
Health Information Exchange that will allow all providers, social services and hospitals to have access to electronic medical records. This project, funded by care providers, hospitals and CCOs in Jackson, Josephine and Klamath counties, will help avoid duplication of medical tests and insure accuracy of medication information.
Expansion of mental health services and capacity for children. Kairos, Jackson County Mental Health and AllCare also support this project.
Prior to joining Jackson Care Connect, Ginger held progressive clinical management responsibilities at Community Health Center, one of Jackson Care Connect’s providers. Most recently, she was director of clinical services and chief nursing officer.
