PORTLAND – Oct. 28, 2015 – Healthcare Resources NW (HRNW) and Humana Inc. (NYSE: HUM), one of the country’s leading health and well-being companies, today announced an expansion of their accountable care agreement to include Oregon Health & Science University (OHSU).
The inclusion of OHSU in Humana and HRNW’s accountable care agreement gives Humana’s Medicare Advantage members access to OHSU Hospital and more than 1,000 physicians. OHSU is Oregon’s only academic health center and recently joined HRNW’s network of health care professionals and providers.
Humana’s existing agreement with HRNW is part of the company’s pay-for-value programs that promote evidence-based, high-quality care with a focus on physician support for successful population health management. The pay-for-value model is designed to improve cost, quality and the patient experience.
The Humana-HRNW accountable care agreement includes value-based incentives tied to performance improvement and supports successful population health management and a world-class patient experience for Humana members in Oregon. The arrangement offers more personalized care and a broad range of population health tools such as Humana’s predictive analytics, chronic care, disease management and wellness programs.

Through its accountable care partnerships, Humana gathers actionable patient data to inform individual care plans and develop best practices in patient care. Value-based incentive arrangements emphasize quality outcomes and leverage standard measures as defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) such as breast cancer screening, colorectal screening, comprehensive diabetes care, and high-risk medications.
Humana has nearly 30 years of accountable care experience. In data released in 2014, Humana members in accountable care relationships experienced fewer emergency room visits and hospital admissions, and improved clinical and management screening compliance for conditions such as diabetes and cardiovascular disease.
