HiMSS Oregon, Population Health in Oregon Forum on November 10, 2015.

The healthcare industry is continuing its evolution from a fee-for-service model to a quality of care model. This trend will only accelerate after the announcement earlier this year by the Department of Health and Human Services plan to tie 50% of Medicare provider payments to value-based models. This enormous shift in care and payment models is helping to drive the growth of population health management. Through the promise of population health, hospitals plan to deliver better care by engaging the patient earlier, providing better care quality, reducing wasteful tests and treatments and enabling better coordinated care.

A critical leg of delivering this value-based care will be the hospitals' infrastructure. Electronic Medical Records are enabling providers to capture new data in pursuit of better patient care but organizations will need to be able to collect and synthesize large amounts of data into actionable improvements in care delivery.

In Oregon Asante, Bay Area Hospital, Mid-Columbia Medical Center, Moda Health, Oregon Health & Science University, Salem Health, Sky Lakes Medical Center, and St. Charles Health System came together and founded Propel Health. By aligning providers, patients and payers, Propel Health offers the tools, technology, and resources to integrate care delivery and enhance the patient experience.
