Healthcare conference in Eugene Highlights Innovations for Improving Patient Care Across Settings

Special event at Lane Community College will feature local and regional experts and guests.

On June 26, 2014, at Lane Community College, Oregon healthcare professionals will convene for a daylong event focused on innovations for improving care and outcomes for patients, and implementing community-based solutions that reduce avoidable hospital readmissions.

The event, “Integrating Care in Oregon Communities: Innovations for Improving Patient Care Across Settings,” will feature local and regional guest speakers, including: Larry Garrett, PhD, MPH, RN, of HealthInsight; Kay Metzger of the Oregon Health Authority; Dan Reece of Peace Health; Roxanne McAnally of the Oregon Home Care Commission; representatives from Acumentra Health, the Medicare Quality Improvement Organization for Oregon; and more.

Media are invited to join regional healthcare stakeholders for what promises to be a conference filled with important industry insights and learning. The agenda includes:

  • A session on how a Coordinated Care Organization (CCO) and a large health system are working together to reduce hospital readmissions.
  • An update on Community Health Worker training and the new roles these workers can play in improving care transitions.
  • Analysis of Oregon’s hospital readmission trends to identify improvement opportunities.
  • The latest on how health systems are using Medicare’s new transitional care billing codes to pay for post-discharge follow-up.
  • Southern Oregon’s communication tools for transferring patients across care settings.
