As one of the state’s new Coordinated Care Organizations, Health Share is responsible for improving the health of the community it serves, including the health of socially and culturally diverse populations. Through a community health assessment, Health Share will identify health disparities associated with race, ethnicity, language, health literacy, age, disability, gender, sexual orientation, geography, occupation and other factors in the Tri-County Metro region. The Council plays an active role in conducting a community health assessment and in designing, implementing and annually updating a community health improvement plan.

About the Community Advisory Council
Comprised of a majority of Oregon Health Plan members, the Council plays a dual role. In addition to advising Health Share’s Board of Directors on the strategic direction of the organization, the Council also enables consumers to take an active role in improving their own health and that of their family and communities.
