FamilyCare Health names Sandra Clark as Director, Population Health

PORTLAND, Ore. (October 14, 2016) – FamilyCare Health has named Sandra Clark as Director, Population Health. In this new role, Clark will provide leadership and direction for the Portland-based CCO’s population health team.

Sandra’s responsibilities include strategizing, planning, implementing, and monitoring innovative quality care programs that improve member’s health outcomes, meet key quality measures, and impact the entire community. Her role is part of FamilyCare Health’s increased focus on impacting health equity and the community determinants of health.

Clark has thirteen years of experience overseeing complex problems, facilitating collaborative efforts, and leading long term strategies to improve health outcomes.
