FamilyCare Health Awards $450,135 to Community Programs

Portland – In line with its mission to Create Healthy Individuals through Innovative Systems, FamilyCare Health today announced it has awarded five grants totaling $450,135. The projects and programs supported by these grants serve diverse groups of Oregon residents and reflect local community priorities.

“FamilyCare continues to invest in programs that improve the health of the individuals and families we serve,” says Jeff Heatherington, CEO and President. “We recognize the importance of our community partners to effect real change and are proud of the collaboration we continue to support.”

The following grants include both new and continuing efforts:

The Quest Center for Integrative Health (QC), $80,000 grant For over 20 years, QC has served those living with life-challenging illnesses, including HIV/AIDS, chronic pain, addiction, and mental health concerns. With the help of this grant, QC will develop the Enhanced Access to Wellness, Integrity, and Sustainable Health Project, an integrated primary medical and behavioral health care project in collaboration with Rose City Urgent Care & Family Practice. This project will give patients a health and wellness home and provide them with coordinated care.

Northwest Family Services, $50,000 grant

Northwest Family Services is known for helping struggling families navigate the web of health, education, counseling, and social services. This grant will help the organization continue its outreach and support of bi-cultural, bi-lingual individuals who struggle with mental illness or drug or alcohol addictions.

Metropolitan Family Services and Wallace Medical Concern, $150,000 grant

Metropolitan Family Services, a non-profit with the mission of lifting people out of poverty, and Wallace Medical Concern, a medical clinic serving the Portland metro area, have joined forces to help eliminate barriers to care and improve health engagement. With this grant, the collaboration will assist newly insured community members—particularly diverse families with young children—establish care, improve health engagement, and learn to better manage their own health.

Outside In, $50,000

Outside In is a local social service agency and healthcare clinic that helps homeless youth and other vulnerable populations in the Portland metro area. These funds will help Outside In complete their migration to a new electronic health records system. This technology upgrade will enhance the delivery of primary care and integrated mental health and substance abuse treatment services to the community members Outside In serves.

Housing with Services/Asian Health and Service Center (AHSC), $120,135

Housing with Services is a collaborative model which provides enhanced health care and social services to the tenants of 11 affordable housing properties for low-income seniors and adults with disabilities. AHSC, as the fiscal agent, serves these individual in concert with other community providers including Cedar Sinai Park, Home Forward, REACH CDC, Cascadia Behavioral Healthcare, LifeWorks NW, Jewish Family and Child Services, and Sinai In-Home Care. This grant will enable the collaboration to increase its navigation/care coordination services throughout the properties.
