To Boldly Go Where No Clinic Has Gone Before

Trillium Community Health Plan, the Coordinated Care Organization in Lane County has awarded grants to local clinics to boldly go where no clinic has gone before by integrating behavioral and physical healthcare in their practices. Integration of these services is recognized as an integral step in providing better care to Oregon Health Plan members and the hallmark of successful transformation. The plan, called the Trillium Integration Incubator Project (TIIP), is designed to encourage a team based approach for delivering primary care and behavioral health services in the same healthcare setting. The funding includes new alternative payment agreements that allow each clinic to be innovative in developing a model that works best for their organization and includes opportunities for in-depth training from national experts as well as a monthly learning collaborative where they are encouraged to share best practices with their peers.

Eight organizations received funding for the project, they are: Oregon Medical Group, Eugene Pediatric Associates/Thrive Behavioral Health Clinic, Springfield Family Physicians, PeaceHealth, The Child Center, Options Counseling, Center for Family Development, Community Health Centers of Lane County/Lane County Behavioral Health, and Willamette Family, Inc. Uniquely, four project sites are primary care medical homes and four project sites are behavioral health medical homes.

With an investment of $535,200 from transformation funding, Trillium will cover the startup and development costs for the project. Lane County through Trillium Behavioral Health provided an additional $250,000.00. Cost savings will come from delivering care in one site, with one team, limiting gaps in care that can lead to a worsening of a person’s mental or physical health and need for more expensive care.  

“We expect that members will receive timelier and more coordinated services with indications of improved health and quality of life,” says Lynnea Lindsey-Pengelly, PhD. “When we offer services in the same location we limit the gaps in treatment that often lead to the deterioration of health requiring more costly services in the future. “

The TIIP program sites have committed to providing innovative integrated care for nearly 17,000 Trillium members.  The goal of the TIIP is to discover the best way to deliver behavioral and physical health services to improve quality of life outcomes for the OHP population and the entire community.  

Trillium, the Coordinated Care Organization in Lane County covers nearly 90,000 Oregon Health Plan members, about 25% of the Lane County population.
