Organization/Company Name:
Yamhill County Health and Human Services
Position Description:

This is a regular full-time position for a Human Services Specialist II or III (Therapist/QMHP) on the Outreach, Assistive Services, and Integrated Supports (OASIS) Team.   Working as part of a small interdisciplinary team, this position provides behavioral health services for adults with mental health conditions and additional complex needs. OASIS serves individuals who have difficulty accessing traditional outpatient services due to medical issues or chronic health conditions, and/or individuals who require a high level of care coordination due to disabilities or special circumstances. When clinically indicated, services are also provided in the outpatient clinic. The OASIS therapist performs a variety of tasks including assessment, service planning, therapy, consultation, and care coordination. Additionally, OASIS therapists are expected to participate in consultation, in-service trainings, and education programs (including training related to working with older adults and adults with disabilities).  They also assist in quality assurance activities and program development as part of a team.
