Mental Health Provider - SALEM Oregon


Oregon Integrated Health is a Tier 4 Patient Centered Primary Care Medical Home.  We have clinic locations in Portland, Salem, Eugene and Florence Oregon.  Our clinics in Portland and Eugene are integrated clinical models with Primary Care Providers that include Naturopathic Physicians, Nurse Practitioners and Medical Doctors, Psychiatric Mental Health Nurse Practitioners, Counselors, Chiropractors, Acupuncture, Physical Therapy, Massage Therapy and Eugene has an Expanded Practice Dental Hygienist for our oral health program.

Our Salem location is hiring for a full time/part time -Mental Health Provider that is interested in working in an integrated medical setting.  Our goal is to expand the access to primary care and integrated services.   All of our clinic locations utilize telemedicine with our patients and we will continue to do so  especially during the COVID-19 outbreak.  Clinic primarily serves Medicaid/Medicare demographic of patients, adults and adolescents.


Provide therapeutic services through assessments using DSM-V and ICD-10 diagnostic criteria.

Mental Health Assessment evaluation with recommendation of Level of Care (LOC)

Assess patient ability and willingness to engage treatment.

Assess and determine patient functional capacity

Documentation of findings using electronic health records in accordance to ethical and legal standards.

Knowledge and use of a variety of therapeutic theories and strategies to best meet the individual needs of the clients served.

This position will work with patients that are currently being seen by Primary Care providers for therapy that will support prevention, early intervention and positive changes for their health.

During the day provider will have 60/30/15 sessions for patients.  Appointment slots will be blocked for urgent walk in, same day emergency follow up as well as sessions are focused on a behavioral changes.  These may include supporting challenging dietary changes for diabetes patients, motivation for changes to movement and improved eating to ensure that the patient has additional support in making behavioral changes that will positively impact their health.


Current, unrestricted license to practice mental health therapy in Oregon ( LPC/LCSW/LMFT).

National Provider Identification number (NPI) prior to start date of employment.


Full Time and Part Time Opportunities available,

Flexible Hours available


$40-$43 an hour
