Northwest Human Services is a non-profit leader in providing advocacy, quality healthcare and social services in Marion and Polk counties since 1970. We are an innovative Federally Qualified Health Center that has been providing care to the local community for over forty-five years. In addition to our Transitional Programs that provide services for the homeless, our medical clinics are Tier V Patient Centered Medical Homes and our West Salem location offers fully integrated primary care, psychiatry, therapy, and dentistry all on the same campus. Our Medical, Dental and Mental Health care teams are committed to the overall wellness of our patients and work together to ensure patient needs are expertly evaluated, and met with compassion and respect.

YOUR ROLE at NWHS:
The BHC is an integral part of the Care Coordination team, working with Medical Providers, Community Health Workers, Nurses, Case Managers, and others to provide continuity of care for patients with complex cases. As our BHC, you will work with patients and clients at our primary care clinics to help them overcome behaviors that are impeding positive outcomes in their medical diagnosis and symptoms. You will accomplish this by providing brief interventions to empower patients with tools and resources to manage their physical and mental health. The BHC also supports outreach efforts of our Homeless Outreach and Advocacy Project (HOAP) through screening, evaluation, and education for A&D, Mental Health, and other needed community resources.

Required Qualifications and Experience: 

* Demonstrated proficiency using DSM IV-TR, DSM V, and ICD-10.
* Successful experience working with vulnerable and underserved populations and clients with dual diagnosis, chronic mental illness, and trauma is required.
* Candidates with experience in a primary care clinic are preferred.

Required Education and / or Certifications: 

* Candidates must have appropriate credentials and level of training including one of the following: 
* A master’s degree in Social Work and licensure as an LCSW in the state of Oregon, OR
* A master’s degree in Counseling and licensure as an LPC in the state of Oregon, OR
* Doctoral level education in Clinical Psychology and appropriate licensing and credentialing.
