Provides direct public health nursing services to the community and provides nursing services for Immunizations, Reproductive Health, Sexually Transmitted Disease, HIV Testing and Counseling, and communicable disease. May coordinate and oversee the Immunization, Tuberculosis and/or perinatal Hepatitis B case management and tracking Program.
Essential Duties and Responsibilities
The following may not be all inclusive and other duties may be assigned:
01 Performs skilled public health nursing services in clinics, homes or other settings to clients in such areas as Immunizations, Reproductive Health, Communicable Disease, HIV testing and counseling, Tuberculosis and perinatal Hepatitis B perinatal Hepatitis B case management and tracking Program. Follows established state guidelines and works under physician's standing orders.
02 Interviews clients, gathers medical history, performs general health screening, prepares client for exam, performs basic exams and provides counseling and teaching of basic health care practices; dispenses medication per physician's standing order; initiates follow-up care and makes referrals to physicians or other community agencies or health resources as necessary.
03 Documents medical care and maintains client medical charts and other necessary records.
04 May obtain lab specimen from clients and provide follow up results to client; may perform lab work for clients and interpret results within guidelines.
05 May provide direct services and educational classes to school students and their parents.
06 May investigate reportable communicable disease cases, complete proper forms and provide appropriate follow up; such as, immunization, referrals, etc.
07 May complete or assist in physical inventory of medical supplies and request additional supplies as required.
08 May collaborate with other community health and social service agencies when necessary to provide optimal health related services.
09 Complies and adheres to Oregon Health Authority Vaccines for Children Program guidelines for Program & Departmental compliance.
10 May be responsible for or assist with timely and accurate completion of school exclusions in accordance with Oregon Health Authority school exclusion laws.
11 Complies and addresses to Oregon Health Authority Tuberculosis Program guidelines;
12 May serve as a resource to community providers for Tuberculosis guidance in consultation with OHA and/or Health Officer and our Standing Orders.
13 Complies with program elements for perinatal Hepatitis B Program
14 May carry emergency phone on a rotational basis.
15 Performs other duties as assigned, provided that the scope of duties falls within this classification.

01 Possession of a current, valid, unencumbered license as a Registered Nurse in the State of Oregon.
02 Possession of or ability to obtain CPR for the Healthcare Worker Certification.
03 Possession of a current, valid Oregon Driver's License and a safe driving record.
Bachelor's degree from four-year College or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
