CCC has two current openings for a RN Care Team Manager!  

The RN Care Team Manager supervises support staff and helps facilitate clinical services within
their own care team and provides nursing assessment and evaluation on other primary care teams.

Clinical Care:

Responsible for nursing assessments, evaluation and documentation; collaborating with
providers and other members of the interdisciplinary health team as needed.

Oversee Medical Assistant (MA) and Health Assistant (HA) including auditing documentation for quality, timeliness and completeness.

Supervises, coaches and arranges training for MA competencies including medication
administration, treatments, procedures and documentation per clinic protocol and in
collaboration with Care Team Manager and Clinical Services Manager.

Team Leadership:

Guides care team in setting and achieving performance goals.
Supervises and/or delegates completion of staff scheduling; including rotating on-call duties

Supervises call management and documentation quality within care team.
Hires, supervises and coaches MA and HA staff, supporting their performance and
development goals.

Quality and Compliance:

Directs innovation in care team and clinic practice through continued quality improvement initiatives.

Required Qualifications and Experience: 

Current Oregon licensure as a Registered Nurse (RN)

Minimum one (1) year of supervisory experience required. Two (2) or more years of
experience preferred.

Minimum one (1) year experience in community health or primary care.

Must meet CCC privileging requirements as required by FTCA.

Must have current CPR certification

Will be required to carry an agency cell phone for work use. Cell phone will be provided by
Central City Concern.

Must pass a pre-employment drug screen, TB test, and background check.
