Financials In Depth: Data Release Shows Charity Care, Margins and Physician Pay at Oregon Hospitals

Over the past decade, Oregon’s hospitals have weathered drastic shifts in how they’re paid, how much they spend, and where they’ve put their money, according to a detailed database released by the state’s Office of Health Analytics in response to a Lund Report records request.

The broad financial data goes back to 2007 for close to 60 Oregon hospitals, and shows that these institutions have steadily increased their spending on physicians and other workers with little influence by the Affordable Care Act. But in other ways, the ACA’s mark is clear, with data revealing that charity care expenses – booked when hospitals reduce or forgive the bill of a patient who cannot pay – dropped significantly once ACA subsidies and marketplaces expanded health insurance coverage in the state.

Likewise, Oregon hospital revenues have shifted since ACA health coverage became widespread at the start of 2014, with far fewer funds coming to hospitals from people who pay their own bills, and far more coming in through Medicaid-funded plans.

To get specific, the statewide figures reveal:

• In the first half of 2016, which is the most recent period covered by the data released on Tuesday, Oregon’s hospitals spent $1.8 billion on non-physician payroll, and $237.2 million on physician payroll.
• Though the sheer dollars spent on non-physician payroll significantly dwarfed spending on physician pay, rising 47 percent since the first half of 2007, during the same time period physician payroll rose by 250 percent. Much of this increase likely reflect a shift toward salaried physicians and away from contracting with physician groups.
• Charity care spending by hospitals peaked at $414.7 million in the first half of 2013, and dropped to 60 percent -- $165.7 million – in the first half of 2016. That represents a slight uptick from Oregon hospitals’ $142.9 million in charity care spending in the first six months of 2015.
• In the first six months of 2013, before the ACA boosted health insurance enrollment, patients without health insurance paid hospitals $590 million – and were billed for $631.2 million in new charges. By the first half of 2016, self-pay patients paid Oregon hospitals only $185.7 million, and were billed for $226.7 million in new charges.

The Oregon Office of Health Analytics hospital finance and utilization trends database contains data that hospitals are required to release to the state, but it’s not comprehensive. It does not include profit and loss statements or cash reserves that could be used to analyze the financial health of a hospital. But it does include a considerable amount of detail. The data also do not include financial results for Kaiser Permanente’s two hospitals, which are fully integrated within the Kaiser system and do not track financials separately.
